Yodel Loss Disclaimer Form - Explained

The loss disclaimer form is required to submit a claim with Yodel, and is mandatory for any claim where parcel tracking includes a delivery scan. If this is not supplied or incorrectly completed, the claim will be automatically rejected by Yodel. The form can be filled out by either the business owner or the customer, but the customer must sign the document before submitting back to HubEurope.

  1. Please open, download or print the form.

  1. Please enter the tracking code. There must be one tracking code per form, so if claiming for multiple parcels, a loss disclaimer form must be supplied for each one.

  1. Please save the form as the tracking code, for traceability when submitting back to HubEurope.

  1. Please include the customer address, email and phone number. This must match the details submitted to Yodel upon label creation, with the exception of destination changes made whilst the parcel was in Yodel's network, in which case please supply the most recently submitted address details.

  1. Please ensure your company name is entered here.

  1. Please cite the date of label generation here.

  1. Please specify the list of goods in the parcel to be claimed for, in as much detail as possible (e.g. Black Size 10 Wool Jumper). If there are more than 5 items in the order, please use the space under the boxes to confirm the other items in the order.

  1. Please print or type the customer's name here.

  1. The end customer must either physically sign, or input a valid electronic signature in this box. Typed or printed names in this box will be considered invalid and will be automatically rejected.

  1. (Optional) add an internal reference for the claim for your own records if you would like.

  1. Download and supply the form with your other claim evidence to HubEurope.
